NON-IMMIGRANT VISA FORMS

Effective November 1, 2006 it has become mandatory for all Non-Immigrant Visa applicants to fill out the Electronic Visa Application Form (EVAF) online.

For inquiries about the EVAF form please click on EVAF Frequently asked questions (FAQ).

To fill out the DS-156 EVAF form, please go to evisaforms.state.gov, required for all applicants.  This form must be completely filled out online.  After pressing "CONTINUE", Applicants need to print out and sign the PDF form. Please do not try to print and fill it out manually; we do not accept manually filled DS-156 forms.


DS-157 Form
Required for all applicants aged 14 and above.  This form should be filled out manually or typed on your computer before the day of the interview.


DS-158 Form
Required for all applicants applying for F, J, & M visas.  This form should be filled out manually or typed on your computer before the day of the interview.
